Spider veins are the dilation of very fine veins that are located in the skin. They are also referred to as telangiectasias (in the case of intradermal veins) or reticular varicosis (in the case of subdermal veins). The problem is usually only of a cosmetic nature, as spider veins do not, by definition, develop into varicose veins.
However, they often occur as an accompanying symptom of a venous disease or as a result of surgery on the venous system. The so-called corona phlebectatica paraplantaris is of important diagnostic relevance. This refers to an accumulation of spider veins below the ankles, which are a warning sign of venous insufficiency. Therefore, in addition to the visual diagnosis, impairments of this kind should always be ruled out.

Foam sclerotherapy

Classically, spider veins and smaller vein dilatations are sclerosed. In Germany, ethoxysclerol with the active ingredient lauromacrogol 400 is the only approved sclerosing agent. When injected, the inner walls of the veins are irritated and made “sticky”, causing them to close up and break down completely. This effect is even stronger with microfoam sclerotherapy, where the ethoxysclerol is foamed beforehand. After the treatment, the legs must be wrapped for a few hours. The method is well tolerated, pigmentation and bruising are rare. If carried out correctly, sclerotherapy is the method of choice for spider veins.

Laser treatment

There is also the somewhat more expensive therapy with the long-pulsed NDYAG laser. The finer the vascular structures, the better the results. However, this method, which requires neither injections nor pressure bandages, is only suitable for spider veins with a diameter of up to 3 mm. The laser light is absorbed by the red blood cells, causing them to swell and close the blood vessel. Pain can be alleviated by a cool air treatment.
